If we have contacted you about a pending application for health care coverage, this means we have worked with you to submit documentation and are waiting for a decision from the processing agency. Please contact the team member who sent you the letter, using the contact information they provided. They can answer additional questions you have about the process.
Your health care provider has a process for obtaining a copy of your medical records. Contact the hospital or clinic to submit a request to them.
