ElevatePFS works with hospitals and health care facilities across the U.S. to help patients pay their medical bills and understand their insurance options. The hospital or health care provider you visited is one of our contracted partners and has asked us to reach out to you to assist with financial care options related to your visit. Please contact us if you believe this to be an error or have questions regarding next steps.
I received a letter and have questions about the status of my application. Who do I contact?
If we have contacted you about a pending application for health care coverage, this means we have worked with you to submit documentation and are waiting for a decision from the processing agency. Please contact the team member who sent you the letter, using the contact information they provided. They can answer additional questions you have about the process.
How do I request a copy of my medical records?
Your health care provider has a process for obtaining a copy of your medical records. Contact the hospital or clinic to submit a request to them.
Am I eligible for disability benefits?
If you have a disability, applying for disability benefits on your own can be difficult. Without assistance, most disability claims are denied. What type of accidents does ElevatePFS help with?
ElevatePFS partners with health systems to investigate the details of your accident so that accident-related insurance pays all, or part, of your hospital bill. ElevatePFS’ role is to gather all accident details and potential sources of insurance that may be responsible for your bill.

These type of accidents could also occur while on the job and may involve a Workers' Compensation claim. If your injury resulted from a work-related accident, complete the online screening form linked in the text or email you received, or call us at 800-544-9925. We will use this information to bill any available insurance.
